Lemek LLC now offers voluntary Accident, Critical Illness and Hospital Indemnity coverages through UnitedHealthcare!
These expanded benefit offerings are available to you, your spouse, and your dependent children to help cover unexpected medical expenses. Employees must enroll in coverage for their spouse and children to obtain coverage. These voluntary benefits can be purchased through post-tax payroll deductions.
The post-tax voluntary benefits may be canceled at any time throughout the year.
Voluntary Accident Insurance
UnitedHealthcare’s supplemental accident insurance provides benefits for covered injuries and specified accident-related expenses for an individual or family. Since health insurance only goes so far, this group policy is designed to help cover the out-of-pocket expenses that can result from a sudden accident.
- Plan covers a wide variety of injuries and accidents, such as: hospitalization, physical therapy, emergency room treatment, doctor’s visits, fractures and dislocations, transportation, lodging, and more.
- Benefits are paid for accidents that occur on or off the job.
- Youth organized sports benefit pays an additional 25% of the total benefit for dependents injured while playing organized sports.
- Plan also offers coverage for accidental death and catastrophic accident.
- Coverage options include employee, spouse and/or children up to age 26.

Voluntary Critical Illness Insurance
Could your bank account survive a serious illness? Be prepared with group critical illness insurance from UnitedHealthcare. Critical Illness insurance can pay a lump sum benefit at the diagnosis of a covered illness. You choose the level of coverage with benefit amounts of $5,000, $10,000, $20,000 or $30,000 for you and your spouse, and $2,500, $5,000, $10,000 or $15,000 for your children. You can use the money to pay for copays, deductibles, childcare, transportation, lodging or any other expenses that you may have. Coverage options include employee, spouse and/or children up to age 26. Rates are calculated during your enrollment session and are based on benefit amount and age.
What is covered? Some of the critical illnesses covered in this plan include:
- Heart attack
- Major organ failure
- Cancer
- Stroke
- Coma
- Paralysis

Voluntary Hospital Indemnity Insurance
A trip to the hospital can be costly – and most people are surprised to learn they are responsible for a good portion of the bill. Hospital Indemnity insurance through UnitedHealthcare provides direct benefit in the event of a hospitalization, regardless of treatment costs or other insurance coverage.
- Hospital admission benefit: $1,500 per calendar year, up to 3 times per plan year
- ICU admission benefit: $1,500 per calendar year, up to 3 times per plan year
- Hospital and/or ICU confinement benefit: $100 per day for up to 364 days
- Health Maintenance Screening benefit: $50
