Want to Earn an Additional $250 HRA Benefit for the next plan year?

Employees have an opportunity to earn an additional $250 in a Health Reimbursement Arrangement (HRA) to use towards medical, pharmacy, dental, and vision related expenses.

Requirement:Employees enrolled in a Lemek sponsored UnitedHealthcare medical plan must complete their annual physical between June 1, 2025 –May 31st, 2026 and submit their Physician Results Form by May 31st, 2026.Your annual physical is covered at 100% within your medical benefit plan.

Rewards: Those employees who complete their annual physical and submit their packet by May 31st, 2026, will receive an additional $250 deposited into their Health Reimbursement Arrangement (HRA) for the 2026 plan year. This means you can receive up to $500 for medical, pharmacy, dental, and vision expenses for Lemek LLC’s 2026 Benefit Plan Year.

Instructions: The Program Guide and Physician Results Form can be found and submitted using the Wellworks For You portal, www.wellworksforyou.com. Please login with the following credential guidelines:

Username: Lemek_ + First Name + Last Name + Year of Birth (YYYY)

Temporary Password: Your date of birth in MMDDYYYY format
