Eligibility

All employees are assessed every six (6) months from their initial Standard Measurement Period (SMP) to determine their eligibility. The measurement periods are:

  • December 1 through May 31
  • June 1 through November 30

During this time employees must ensure they average 30 hours or more per week to maintain their health benefits.

Open Enrollment is June 17th - 28th, 2024

During this time, you can:

  • Change your current coverage, if needed
  • Enroll in benefits, if newly hired or newly eligible for benefits
  • Update your dependent information
  • Update your beneficiary information

You will enroll in benefits through your UKG Pro benefits portal or the UKG Pro Mobile App.

Qualifying Life Event

Certain life events provide you a 30-day window to elect benefits or change your covered dependents.

What is a Qualifying Life Event? If you experience any of the scenarios below, you may be eligible to update your benefit elections mid-year:

Qualified Life Event Documentation Needed
Change in marital status • Marriage

• Divorce/Legal Separation

• Death
• Copy of marriage certificate

• Copy of divorce decree

• Copy of death certificate
Change in number of dependents • Birth or Adoption

• Step-child

• Death
• Copy of birth certificate or copy of legal adoption papers and Social Security card

• Copy of birth certificate plus a copy of the marriage certificate between employee and spouse and Social Security card

• Copy of death certificate
Loss or Gain of Other Coverage • Loss of or enrollment in Medicaid/Medicare/State Plans

• Change in spouse’s or dependent’s benefits or employment status
• Confirmation Statement confirming benefits enrolled, enrollment dates as well as individuals covered

• Notification of spouse’s employment status that results in a loss or gain of coverage
